PersonLink is a supports coordination organization (SCO) serving individuals with intellectual disabilities in the greater Philadelphia region. Founded in 2004, PersonLink contracts with the Pennsylvania Department of Public Welfare's Office of Developmental Programs to support individuals and families in Philadelphia, Montgomery, Delaware, and Chester Counties. Currently, we are supporting more than 1,300 individuals.
Our compassionate Supports Coordinators utilize a person-centered planning approach and are trained to work collaboratively with individuals, their families, and service providers to develop an Individual Support Plan (ISP) that addresses the specific needs and desired outcomes of the individual. We assist these individuals in locating, coordinating, and monitoring the community resources they need to live the fullest, healthiest lives possible.
PersonLink's mission is to provide quality supports coordination to individuals who are diagnosed with an intellectual disability using a holistic approach that encourages dignity and self-fulfillment.
