The role of the Supports Coordinator is to:
Here at PersonLink, our Supports Coordinators are ready and able to meet individuals at a day program or provider, in their home, or in the community. We are committed to being responsive to the needs of our individuals; our staff returns phone calls within 48 hours should you have any questions or should any issue arise with your services.
We believe that having a Supports Coordinator is an incredibly valuable tool in assisting individuals with intellectual disabilities to live the fullest, most meaningful lives possible. PersonLink Supports Coordinators are mediators and friends in a system that can be confusing to navigate alone. We look forward to working with you to achieve your or your loved one's goals.
Family Support Services (FSS) is a grant program that helps the individuals we serve access additional resources to enhance their home and community lives. Our Supports Coordinators are able to assist individuals in the annual application process. FSS funds can be applied toward services such as:
At PersonLink, we speak your language. Literally.
If you are a Limited English Proficient (LEP) family or household, PersonLink is committed to serving you in your language. All phone calls will include an interpreter on the line, all in-person appointments will have an interpreter present, and all written correspondence and documents requested will be made available in the family’s native language and translated by a certified translator, rather than by online automatic translation.
Our commitment is rooted in two federal laws: Title VI of the Civil Rights Act of 1964 and Executive Order 13166 (EO 13166), "Improving Access to Services for Persons with Limited English Proficiency."
Our Restorative Justice Supports Coordination Program (RJSCP) is a highly trained unit that provides supports coordination to individuals with intellectual and developmental distinctions (IDD) and autism who are involved with the criminal justice system. The program seeks to repair the negative impact caused by an incident while balancing the needs and roles of the offender, the victim and the community. It works within the framework of trauma informed care, recognizing the importance of physical, psychological and emotional safety of the individual while also expecting accountability as a main aspect of restorative justice. Through this lens, we offer assistance to the courts in an effort to link individuals with IDD to services in the community as an alternative to incarceration.
Further, we provide awareness education and supports to help individuals and their families navigate the criminal justice system, working toward ending continued criminal involvement.
To choose PersonLink as your Supports Coordination agency, you must register with your County Administrative Entity.
